If you choose an avatar without considering how much it might upset others then you run a higher risk than others. If you specifically pick one to upset others you stand a very high risk.

We've also got a few different people moderating. I may see someone's avatar and not take an action but that doesn't mean it's been approved. integral could decide otherwise 30 minutes later and that is fine because I may have been undecided or simply not noticed when I saw it. It's no big deal and we'll usually just make a friendly request anyhow - there's millions of things you could choose so pick something else.
