I know this can happen once in a while, but it seems that recently, it's been happening much more frequently. Attackers seem to be losing more often, even with a 2-3 dice advantage.

Has the dice-rolling code been changed in the past month or so, to give attackers lower rolls (or defenders higher ones)?
